Pediatric prophy progress notes:

Patient presents 10 mins late for prophy appt.
HHX: Reviewed with parent.
OHI: Dislosed, showed patient how to use modified Bass technique and C-shaped flossing. Patient then demonstrated. Review diet and health snacks and drinks.
Generalized moderate plaque, generalized light calculus with localized moderate calculus on the lower anteriors, localized moderate bleeding lower anteriors. Scaled to completion, polished and flossed.
Applied 5% sodium fluoride varnish.
Patient was talkative, liked to hold the suction, and cooperative.

Pediatric prophy progess notes (for patient under the age of 3)

Patient presents on time for prophy appt.
HHX: Reviewed with parent.
OHI: Reviewed anticipatory guidance with mother (bottle use, sippy cup, healthy snacks, brushing 2X day and flossing 1X day). Showed mother knee-to-knee technique for brushing and flossing. Showed mother frenum area and to be cautious when brushing, lift the lip technique and use of floss holders.
Performed a TB prophy in the knee-to-knee position and flossed.
Applied 5% sodium fluoride varnish.
Patient was very uncooperative and cried.
